Insurance & Billing

Insurance Cards & Identification: Please bring your current insurance cards and photo identification to every visit. We must verify your coverage to ensure accurate billing and processing of your claims. If your insurance changes, notify our office immediately.

Referrals & Authorizations: Many insurance plans require a formal referral from your primary care physician before seeing a specialist. It is the patient's responsibility to ensure that all necessary authorizations are obtained prior to the consultation or procedure. Failure to do so may result in your insurance company denying coverage for the visit.
